In 1993, Brandon Lee was on the set of The Crow, his first and ultimately final starring role, filming an action scene. In the middle of a stunt, a gun that was loaded with blank cartridges, suffered a malfunction. the spent blank casing from shot 1 was jammed into the barrel. Upon using the gun again for shot 2 the second blank went off and propelled the cartridge from shot 1 forward, hitting Lee directly. He was rushed to the hospital and died later that day.
