The late Brandon Lee.

In 1993, Brandon Lee was on the set of The Crow, his first and ultimately final starring role, filming an action scene. In the middle of a stunt a gun that was thought to not be loaded accidentally went off, hitting Lee directly. He was rushed to the hospital and died later on that day.

His death was caught on camera and allegedly destroyed under the insistence of director Alex Proyas. Some of the cameramen claim otherwise, though, one even claiming he still owns a copy of the footage. Either way, nothing from this footage has ever surfaced, the closest thing being a photograph of Lee's face, taken post-mortem, covered in blood.[1]
